Insignia are prefix upgrade components applied to armor that are available for armor sets crafted in Elona and also in the PvP Equipment panel. Insignia provide bonuses to armor sets. For example, the Radiant insignia adds extra energy in addition to the base statistics of the armor.

Word usage (from Dictionary.com):

INSIGNIA, originally the plural of Latin insigne, began to be used as a singular in the 18th century, and the plural INSIGNIAS appeared shortly thereafter. All uses—INSIGNIA as a singular or plural and INSIGNIAS as a plural—are fully standard. The singular INSIGNE still occurs, but INSIGNIA is more common.

In campaigns prior to Guild Wars Nightfall, prefix bonuses are inherent in certain armor sets. By separating the prefix from the armor set, the players can now have any of the insignia bonuses on any post-Nightfall armor set they choose. This allows for the combination of any functionality with any post-Nightfall armor art. Insignia are the prefix on a piece of armor, so you can also have a rune (suffix) on the same piece of armor. Some insignia will have different bonuses when applied to different areas of the body.

It is important to note that insignia can not be applied to armor purchased in Tyria or Cantha, or to Obsidian Armor crafted with built-in prefixes. This is because all armor in Cantha and Tyria has the 'prefix' already added when you buy it. Also, you cannot apply Insignia that is of another Primary Profession.

For example, Druid's Armor for the Ranger has the energy bonus you would receive from adding a Radiant insignia to a blank Elonian armor. Insignia can be applied to blank Obsidian Armor crafted after the October 25th update. Armor of heroes acquired outside Elona will also accept insignia. e.g. Olias (Necromancer) and Zenmai (Assassin)

Insignia are available from Rune Traders in all campaigns, and can be traded with other players, however the only way to unlock new Insignia in PvE is to salvage them from 'salvage armor' obtained as loot. PvP characters can create armor with Insignia in the PvP Equipment panel regardless of campaign, like all PvP items, this armor cannot be salvaged.

Like Runes, Insignia have fixed value bonuses - there are no "non-max" Insignia.

So far, Bloodstained Insignia have only been observed on purple and gold items, and Lieutenant's Insignia have only been observed on gold items.

Bug!
The Sentry's Insignia does not work at all. After testing with a ranger wearing a full set of armor with this insignia, Lightning Orbs from Isle of the Nameless' Master of Lightning deal 78 damage regardless if any stance is being used or not. While wearing an enchanter's armor, the same lightning orb deals less than 70 damage per hit when its conditions are met. Additional testing with Sand Wurm in normal mode also showed the same result: 139 damage criticals hit with full sentry's insignia when in a stance, and 117 damage critical hits with full Blessed insignia when under the effect of enchantments.
